All campers are required to complete a brief medical history statement and standard minor consent form signed by a parent or legal guardian prior to any pool sessions.

If any medical questions are answered “Yes,” clearance from a licensed physician is strictly required before your child can participate in scuba activities. Please review these safety forms early to ensure a smooth start to camp.
